For this assignment you will be creating a Microsoft Word document presentation that shows applying the Zero Trends Model (book by Dee Edington) – for the CDC Comprehensive Workplace Health Programs to Address Physical Activity, Nutrition, and Tobacco Use in the Workplace the pre-selected program.

1. Structure a Zero Trends Program Evaluation Model following the book to demonstrate how the organization would apply the Zero Trends Model. Present the Mission, Business Case, and Solution. For the solutions section address all five pillars. Add a section at the beginning titled Executive Summary and at the end add a Summary and Conclusions.
Keep in mind the focus and rationale for presenting and using the Zero Trends Model is on changing the level of thinking within your chosen organization. You have been given the opportunity by top leadership with HR and your organization to describe how the organization and you in your role as a health promotion program manager will be addressing health care costs, performance and also the well-being of the workforce applying the Zero Trends approach.
Your goal for this project is to develop a plan model that shifts the focus from waiting for employees to get sick to suggesting fixes within the environment, culture and lifestyle systems visible within the organization at this time that have led to poor performance and high costs. Collect and use data if available to support your ideas using the CDC checklist evaluation section outline.
Expectations and Formatting of Paper
• The recommended length of the plan is 8 pages in length, business professional tone – supported with one table or chart.
• Table of contents,an Executive Summary, Section Headings from the text and other sources, Summary and Conclusions and References.
• Insert page number in the top right of each page excluding the title page.
• Add an Appendix for raw data that is too long to insert within the document.
